Job Description

SCHEDULE: (Full-Time) Monday-Friday (8 AM - 3:30 PM)

General Duties:

• Will comply with all agency policies & procedures
• Will advocate for the health & wellbeing of all program consumers
• Will report any unsafe or hazardous condition immediately
• Will report any suspected neglect or abuse immediately
• Will complete required training hours
• Will work hours as stated on the job description
• Will perform other duties as assigned by supervisor/manager

Program Specific Duties:

• Will be responsible for job development & job placement
• Will plan & implement job specific skill training
• Will assess areas of need & interest for assigned consumers
• Will provide assistance, training & supervision to assist the consumer in adapting to the workplace
• Will provide job counseling to assigned consumers
• Will assist with job related transportation arrangements
• Will actively market this service in a professional manner
• Will complete Community Based Work Assessments
• Will receive referrals from OVR, SAM & Ticket to Work; & keep OCR, other funding sources & Ticket to Work updated on progress of placements monthly.
• Will visit Career Link on a regular basis to keep informed of job vacancies, job fairs, etc.
• Will meet with each client bi-monthly
• Will provide follow up to individuals who have been placed into competitive jobs
• Will prepare & submit billing for Supported Employment to the appropriate agencies
• Will provide assistance with Home & Community Habilitation scheduling & paperwork when necessary
• Travel between programs & other organizations on an as needed basis or directed by supervisor/manager

Transition Services:

• Will work with local school districts related to school to work transition activities
• Will participate in the Transition Council, Employability Fair & Transition Fair
• Will conduct student assessments & inform families/parents about Avenue's services
• Will refer students to SAM or OVR, whichever agency is appropriate

Requirements:

• Acceptable results on all pre-employment testing & background checks
• Ability to comply with all physical demands of the job, with or without reasonable accommodations
• Valid PA Driver's License, acceptable driving record, access to a reliable vehicle with proof of auto insurance & auto registration
• Bachelor's Degree & 2 years applicable work experience or High School Diploma & 4 years of applicable work experience or any equivalent combination of education & experience
• Ability to work independently
