Job Description

Scion Staffing has been engaged to conduct a search for a Social Worker for an organization dedicated to improving the well-being of individuals living with dementia and related conditions. This position is onsite in a supportive, community-driven environment on Long Island.

POSITION OVERVIEW

The Social Worker will offer compassionate support and resources to individuals and families navigating Alzheimer’s and Dementia. This position involves conducting assessments, developing safety and intervention plans, and leading educational programs for caregivers. The ideal candidate will work closely with community members to ensure they have access to the necessary resources and support.

RESPONSIBILITIES

• Conduct assessments and intake interviews to understand the needs of individuals affected by Alzheimer’s and Dementia, and develop personalized safety and intervention plans.
• Provide direct counseling and resource referrals to clients through in-person meetings, phone calls, and online communication methods.
• Lead educational programs for caregivers, helping them navigate the complexities of Alzheimer’s and Dementia care with expert guidance.
• Facilitate support groups for caregivers, offering a space for shared learning and emotional support.
• Represent the organization at external events and conferences, expanding its presence in the community and field.
• Assist with special projects and other duties as assigned within the scope of the role.

QUALIFICATIONS

• NY State Licensed Social Worker (or related qualification).
• Strong interpersonal, communication, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Proficiency with MS Office applications (Word, Excel, PowerPoint, Outlook).
• Experience in providing support to individuals and families affected by Alzheimer’s and Dementia is a plus.

COMPENSATION AND BENEFITS

• Annual salary up to $68,000, plus comprehensive Benefits Package including health, dental, and vision coverage.
• Onsite work schedule – Monday-Friday, 9 am – 5 pm.
