Immediate Opportunity LPN Intake Coordinator Home Health

Location: Bronx, NY

Job Type: FTE/Permanent

We are seeking a dedicated LPN Intake Coordinator to join our Certified Home Health Agency. This role plays a vital part in ensuring safe and timely patient transfers, admissions, and care coordination.

Key Responsibilities:

• Manage the referral, screening, evaluation, and admission process for CHHA patients.
• Coordinate scheduling of Nurse Care Managers and vendors for timely evaluations.
• Work with leadership to establish new referral sources.
• Communicate clearly with patients, families, and healthcare providers.
• Perform on-call duties and other assigned responsibilities.

Qualifications:

• Current NYS LPN License, graduate of an accredited nursing program.
• Minimum 2 years’ experience in hospital or home care, including discharge planning.
• Strong multitasking, prioritization, and communication skills.
• Knowledge of homecare regulations and practices.
• PRI assessment experience preferred.
• Valid driver’s license; proficiency in MS Office.

Compensation & Benefits:

• Salary: $80,000 – $85,000 annually
