• Conduct comprehensive social work assessments to determine the needs of patients and their families regarding post-discharge care and support.
• Develop and implement individualized discharge plans that address the medical, social, financial, and emotional needs of patients.
• Provide counseling and support to patients and families related to coping with illness, hospitalization, and transition to post-hospital care.
• Coordinate and collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure seamless discharge planning.
• Facilitate referrals to community services, outpatient care, and long-term care facilities as needed.
• Advocate for patients and families to ensure their needs are met in the discharge process.
• Ensure compliance with hospital policies, procedures, and regulatory requirements in all aspects of discharge planning.
• Maintain accurate and up-to-date documentation of social work services provided.
• Provide education to patients and families about community resources, insurance, and other relevant topics to support the discharge process.

Skills Required

• One-year LMSW experience
• Recent Hospital Experience required

Education/Training/Certifications

• Minimum Degree Required: Master's Degree
• Valid Licensed Master of Social Work (LMSW), license issued by NYSED, Primary Source Verification required.

Additional Requirements

• Malpractice Insurance - All Credentialed Providers are required to have active individual Medical Malpractice.
• Schedule Notes: 9:00 AM – 5:00 PM
• Local candidates preferred
